This page summarizes how protected health information (PHI) is handled for therapy clients of Sophia Alexis. Therapy is a licensed clinical service, and client health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and Georgia law. Coaching clients are not clinical clients, but their information is still treated with the same care and discretion.
The complete, formal Notice of Privacy Practices is provided to every therapy client before services begin and is available at any time on request.
How health information may be used
- Treatment: to provide, coordinate, and manage your care.
- Payment: to bill and collect payment for services, with the minimum necessary information.
- Health-care operations: limited internal purposes such as record-keeping and quality of care.
Beyond these purposes, your information is disclosed only with your written authorization or where the law requires it—for example, to prevent serious harm to you or others, in cases of suspected abuse or neglect of a child or vulnerable adult, or in response to certain court orders. We will always disclose the minimum necessary.
Your rights
- To receive a copy of the full Notice of Privacy Practices
- To inspect and request a copy of your records, with limited exceptions
- To request amendments to your records
- To request restrictions on certain uses and disclosures
- To request confidential communications (for example, a preferred phone number or email)
- To receive an accounting of certain disclosures
- To file a complaint—with us directly, or with the U.S. Department of Health and Human Services—without retaliation
